Media Release, Acknowledgement of Risk & Liability, Policy Agreement:
I give my consent to be included in any promotional materials that may be used for Elite Dance Force Studio (videos/photos) & to be added to the studio mailing list for communication purposes.
I understand the risk of personal injury associated with participation in activities at Elite Dance Force. I agree to waive any and all claims of liability, and hold Elite Dance Force harmless if such injury does occur. If a parent/emergency contact cannot be reached in the event of such injury, I give Elite Dance Force permission to provide medical attention.
I have read & agree to be bound by the studio Policies regarding conduct & payment (located on the studio website).
